Instructions on Applying:
All applicants must answer every question on the Summer Camp Application. Incomplete applications will not be accepted. You are required to include your child’s Social Security Number OR his/her10-digit Florida ID number. (If your child does not have a SSN and you do not know your child’s 10-Digit Florida ID Number, it can be found on your child’s FCAT results or by asking a representative from your child’s school.) Please also provide all of the required signatures on the reverse side of the application.
If your child attends any of our feeder schools: Eneida M. Hartner Elementary, Frederick Douglass Elementary, Frances S. Tucker Elementary, Riviera Middle, Tropical Elementary, Gulfstream Elementary, or Liberty City Elementary, do NOT download this application. You must inquire within your school’s Shake-A-Leg Liaison for an application.
If your child has a documented disability, you are required to fill out the Participant Needs Survey. You are also required to submit a copy of your child’s IEP, Section 504 Plan, medical diagnosis, or other official documentation of disability with the application. There are no exceptions.
If your child does not have a documented disability, does not attend a Title I school, and does not receive a free or reduced lunch, click on "Scholarship Opportunities" above to read more.
Please print out the Application(s), fill it out in blue or black ink and mail, fax (305-858-6262), or deliver to Shake-A-Leg Miami, Attention: Summer Camp Registrar. Incomplete applications will not be accepted.
APPLICATIONS WILL BE ACCEPTED NOW THROUGH FRIDAY APRIL 3RD. AT THAT TIME REGISTRATION WILL CLOSE, UNLESS OTHERWISE DECIDED BY THE SHAKE-A-LEG SUMMER CAMP TEAM.
