2009 Membership forms

     We are now collecting membership dues for 2009.  First of all, there are two ways for you to register and pay your dues.  The first method is for you to "Register & Pay" on line.  The second method is our traditional method (PDF) of "Print & Mail."  If you choose the print & mail method, everything is completed with one form.  If you choose the on line method, of register and pay, your registration is not complete until you submit your medical information for 2009. The medical form is a (PDF) "Print & Mail" form.

     Which ever method you choose, you still have several options for your membership.

               Option #1 - Platinum Card:  $148.00   This card includes the $15.00 for dues, and a discounted rate for all 5 ordeals, as well as a discounted rate for the Annual dinner, Pow-Wow, Fall Fellowship, and LLDC.  This card must be purchased by December 15, 2008.  The purchase of this card will include a $12.00 gift certificate, good for "MikiWare" at the Lodge Trading Post.

               Option #2 - Gold Card:  $123.00   This card includes the $15.00 for dues, and a discounted rate for all 5 ordeals, as well as a discounted rate for the Pow-Wow, Fall Fellowship, and LLDC.  This card must be purchased by February 15, 2009.  The purchase of this card will include a $12.00 gift certificate, good for "MikiWare" at the Lodge Trading Post.  Gift certificate expires March 31, 2009

               Option #3 - Silver Card:  $83.00   This card includes the $15.00 for dues, and a discounted rate for all 5 ordeals, as well as a discounted rate for Fall Fellowship, and LLDC.  This card must be purchased by February 15, 2009.  The purchase of this card will include a $5.00 gift certificate, good for "MikiWare" at the Lodge Trading Post.  Gift certificate expires March 31, 2009

               Option #4 - Standard Dues:   $15.00/year.  The payment of dues only, by March 1, 2009, will entitle the purchaser to a $5.00 gift certificate, good for "MikiWare" at the Lodge Trading Post. Gift certificate expires March 31, 2009 All dues paid after March 1, 2009 do not receive any discount or gift certificate.
