Heart2Heart Health is the prevention component of the Heart2Heart national pilot. Experience is the best teacher.  Those who have been affected by cardiovascular disease are the best people to lend support to those who have just been diagnosed or are at risk for heart disease and stroke.

Join online as a participant and see what the American Heart Association/American Stroke Association has to offer.  You can find a Heart Guides in your area who can offer you one-on-one support to help you reach your health goals, translate terminology, and find American Heart Association/American Stroke Association and other local resources.  We have trained Heart Guides waiting to connect with you to offer support on a number of topics.
